If your school or organization is tax exempt, please include your tax exempt certificate or number. If your school or organization is not tax exempt, please indicate your tax rate. Please send your order form via fax 801-289-0188 or by email lleary@fairviewlearning.com. Once we receive your order form we will get an estimate created and sent over for approval.
